Health coverage that Here's What's Covered: Student Select coverage is created for today’s undergraduate and graduate college students. It covers you against unexpected illnesses or accidents. It provides for high cost items such as hospital stays and surgery as well as everyday occurrences like doctor visits. Student Select allows you to pick your hospitals and physicians, covering your medical care in hospitals, at freestanding ambulatory surgical centers, or at urgent care centers. More specifically Student Select provides... Physician office visits for illness or injury. Physician services including inpatient and emergency room care, and surgical care including anesthesia services and postoperative care. X-rays and laboratory services for outpatient x-ray, radioactive treatment and laboratory services. Covered services include one screening mammogram per calendar year for females 35 years or over. Emergency care including ground and air ambulance to the nearest hospital able to treat the illness or injury. $1 million lifetime protection - $100,000 per illness or injury. Medical evacuation when medically necessary after illness or injury resulting in hospital admission. Evacuation will be to home country or to a facility operating within the laws and standards of home country. Coverage is up to $10,000 per lifetime. Semi-private hospital room rate with no limit on days. Intensive care and coronary care units with no limit on days. Organ transplants including lung, heart, heart/lung, liver, kidney, cornea, and skin. Lifetime maximum benefit of $100,000 for services from donor search through follow-up care for 12 months post transplantation. Inpatient and outpatient surgery including reconstructive surgery for an illness or injury commencing while covered under the policy. Physical medicine for outpatient treatment of vertebrae, disc, spine, back, neck and adjacent tissues up to $750 per calendar year. Home health care including services administered during a home health care visit such as nursing care, intravenous drug therapy, and occupational/physical/ respiratory/speech therapies. Coverage is up to 160 hours of visits per calendar year. A visit can be from 15 minutes up to four hours. Medical equipment and supplies such as crutches, wheelchair and oxygen. Rehabilitation and skilled nursing facility charges for up to 30 days following illness, injury or hospitalization. Temporomandibular (TMJ) or Craniomandibular (CMJ) Joint Dysfunction services including diagnostic exam and x-ray, injections, physical therapy and surgical treatment up to $1000 per lifetime. Complications of pregnancy including miscarriage, ectopic pregnancy and spontaneous premature delivery of a nonviable fetus if the pregnancy begins and ends while the covered person is covered under Student Select. Also covered are medical conditions whose diagnoses are distinct from pregnancy but are adversely affected by pregnancy such as diabetes and chronic hypertension. Repatriation benefit which covers preparation and transportation of the insured’s remains to his or her home country or country of regular domicile if death occurs while insured, and cause of death is a covered illness or accident. The repatriation benefit is $10,000 over and above any other maximum benefit amounts. Additional benefits that your policy will clearly explain. Got questions? We have the answers! This section provides you with the answers to the most commonly asked questions about our Student Select plan. Frequently Asked Questions - General Information Q. When does coverage begin? A. Coverage can begin as soon as tomorrow if you apply today and request today as your policy "effective date". Coverage is effective the later of the following dates: 12:01 a.m. on the date you request, or 12:01 a.m. on the date following the postmark affixed by the U.S. Postal Service on the envelope containing your application - whichever is later. Q. How many times may I renew my plan? A. Student Select is renewable as long as you need it...and, of course, pay the premiums. This is important after graduation while looking for a job. It's hard to find a college with a health plan that stays with you. Q. How long after I submit my application will I receive confirmation or acceptance? A. The Specialty Products Department at Fortis Health will review your application for eligibility and check the submitted premium. Most eligibility determinations are made within 48 hours of receipt. It is important that we have a current phone number should we need to verify any information. If your application is complete and the full premium is paid, your identification card and contract will be sent to the "correspondence address" indicated within 72 hours of receipt. IMPORTANT: IF YOU ARE REQUIRED TO PROVIDE PROOF OF INSURANCE TO YOUR SCHOOL, PLEASE SUBMIT YOUR APPLICATION AT LEAST TWO WEEKS BEFORE PROOF OF COVERAGE IS NEEDED. Q. Even if I graduate, is this plan still effective? A. The plan is effective through the end of the 12-month policy period. Then you can renew it as long as you need it...and, of course, as long as you pay your premiums. Q. Why is Student Select not available in all states? A. Student Select has not been filed and approved in Georgia, Kentucky, Maine, Minnesota, New Hampshire, New Jersey, New York, Oregon, South Carolina, Hawaii, and Alaska. We are unable to issue you a Student Select policy if you are a resident of one of these states and you also live in one of these states while attending school. Q. Do I get a refund? A. Student Select guarantees your satisfaction. You get a full refund if you return the policy within the first ten days. You may also cancel with written notice at any time during the policy and receive a refund as described in the policy. Frequently Asked Questions - Eligibility Q. Who is considered an eligible Student? A. An eligible undergraduate student is defined as one carrying at least nine credit hours at an accredited college or university. An eligible graduate student must meet the graduate student guidelines of their college or university for full-time student status. Q. The school I am attending operates on a quarterly schedule. Full-time undergraduate status is six to eight credits per quarter. Do I qualify for Student Select? A. Yes. Tell us in writing (attached to the application) that you are attending a school with a quarterly class schedule and that you meet the school's definition of full-time status. Q. I am a foreign exchange student. Am I eligible for Student Select coverage? A. Yes. As long as you are an eligible student as defined above. Q. May I add my family (spouse and dependents) to my policy? A. The Student Select plan is designed for individual college students only. Student Select does, however, comply with any state requirements for newborn coverage. When can I take out a policy? I would like to apply now but won't be a student for another month. A. You can apply as soon as you are enrolled to be an eligible student. Frequently Asked Questions - Benefits Q. Are there additional benefits available? A. Maternity benefits are available in most states for an additional charge. Additional benefits such as psychiatric, substance abuse/mental illness, alcohol, rehabilitation vary by state. E-mail or call us at 1-800-341-3534 if you would like additional information. Q. I have had an on-going health problem since childhood. My current insurance plan covers my condition. It appears I qualify for coverage under Student Select since I meet the definition of an eligible student. Will my current medical problems be covered by Student Select? A. Individuals should always be cautious when considering a health insurance change. The benefits and risks of changing plans should be balanced against each individual situation. While Student Select may offer a more affordable premium or higher maximum benefits for your condition, in most cases pre-existing conditions will not be covered until Student Select has been in effect for 12 months. Pre-existing conditions are those for which individuals received medical advice, diagnosis, treatment or services from a physician within the 12-month period prior to the covered person's effective date. Student Select does comply with all state requirements for preexisting conditions and for continuity of coverage. We would be happy to discuss your situation with you. Please call us at 1-800-341-3534. Q. Does Student Select cover me when I go out of state? A. Student Select travels with you anywhere in the United States, its possessions, and Canada. And, although it does not cover you in a foreign country, Student Select does cover you for an emergency medical evacuation to your home country or a facility operating within its laws. Q. Why is there no prescription drug coverage? A. Prescribed drugs are covered if you are hospitalized as an inpatient. Since, outpatient drugs are provided through most university health centers, they are not a covered benefit with Student Select. This helps keep the plan affordable. Q. How often does my deductible start over? A. Student Select's deductible is a 'calendar-year deductible'. This means that it starts over again on the first day of each new calendar year. We realize that you are less likely to meet your deductible and have us pay benefits if you get ill or get hurt in the last few months of a year. That is why Student Select includes a 'carryover deductible' feature. The 'carryover' feature looks at any covered charges you have in October, November or December of any year that go toward meeting your deductible. Fortis Health will 'carry over' the full amount of these charges to the next year to help you meet your new calendar-year deductible. Q. How does Student Select pay my benefits? A. When applying for coverage, you select a calendar-year deductible amount which you pay first. $250 or $500 or $1000 or $2500 Once you meet your deductible amount, Student Select pays 80% of the next $10,000 of covered charges each year. $8000 paid by Fortis
Insurance Company Then, Student Select pays 100% of your remaining covered expenses for the remainder of the calendar-year, up to $100,000 for each illness and injury and up to $1 million total plan maximum. 100% paid by Fortis Insurance Company At the beginning of each calendar year, your deductible starts over. Any payments you make during October, November and December that go toward meeting your deductible that year are carried over by Student Select to satisfy your deductible for the new calendar year. This carryover deductible helps you maximize your late-year health care payments.
